If I want to gift a game to someone what is the process?

Must they create an account or can I just purchase it and download the file/s for informal distribution?

I am sure this question has been asked before but now everyone has a new opportunity to make a scintillating comment!

Legally, they should make an account and redeem the code.
Technically, you could buy the game yourself, download the installer and then give them the file(s), and any subsequent updates if/when they come, and also handle any support requests they may have. From a legal standpoint, that's "piracy", but if you don't also install the game yourself and it's still one purchase, one user, I wouldn't call it wrong, and doubt anyone'd really care.
Yes, as Cavalary said, it is legally not allowed.

The reason is that when you purchase a game, you are not buying the game files - you are buying a license to use the software. The license you purchase on GOG is tied to the service agreement you also accept when using GOG's platform, and that is is linked to an account.

You can gift a game code and explain they will need to set up an account to reclaim it, so no need to have the account before receiving the gift.
Note that some games require Galaxy to do multiplayer, so depending on the game the account may still be required unless the choice of game is sufficiently free.
It's 100% legal to create that account for that someone, buy the game on that account with your card, and give them the game (+ account).

I did that when my wife and I started to play Stardew Valley.

And, who's to say you can't be that persons administrator of that account if need be? I do that that with some of my parents accounts.

That's an excellent point. Would still need a separate e-mail address, but no reason why it couldn't be one made just for this purpose that you access instead of the recipient, as long as you'll maintain access to it.

"3.3 Your GOG account and GOG content are personal to you and cannot be shared with, sold, gifted or transferred to anyone else. Your access to and use of them is subject to GOG’s Privacy Policy and Code of Conduct which are updated or amended when necessary."
