There's a single level one in Kane & Lynch where you have to "let your crew do the work" but that was never really explained. I mostly hid behind a pillar and suppressed and in one of my playthroughs I got it. Exactly what the conditions were is a mystery but I'm guessing you have you finish the level with the lowest kill count
