Save for alternate costumes that I presume just switch around skins on characters when they're enabled, how well are the free pieces of DLC integrated into the game? Are the quests and items just dumped on you when the game starts, or are they organically placed around the game world?

You purchase the Temerian Armour, Nilfgaardian Armour, Undvik (Skellige) Armour, and Crossbow set. The Wolf Gear is an added scavenger hunt. Quests have to be obtained like any other.

The "alternative look for $name" DLC just automatically appears on the women when it is appropriate attire, which is "most of the time" for the most part. The armour is like any other armour in the game insofar as you see it when you have it in your inventory and decide to wear it. You have to buy it before you can do that though which means knowing who sells it - which are basically the most predominant smiths in each region. The custom witcher gear shows up as scavenger hunts like all the other Witcher gear and is found the same way as the built-in gear by discovering the quests by finding letters/maps etc. or buying them as appropriate, then doing the treasure quests. The regular quests drop-in just as all other optional quests in game, they show up on a notice board causing it to turn yellow, so you just look at all the maps and find the yellow notice boards then go there to activate the quest.

The shave and a haircut DLC is available when you find a barber shop and go there. Most of them are found in the major cities and a few larger towns throughout the game and show up as a pair of scissors on the map. The drunken barber in Novigrad slums will give you a bad haircut and tell you to take a hike too. :)
