Without spoiling much, an important dude gets his head crushed and is choked to death. At this point a battalion of police should spawn in, which of course does not happen. Furthermore everybody just accepts that the guy was "poisoned" by the word of a single person who is not a doctor? With the amount of hardware in his body the would be a least some indication about his cause of death (as information for the incoming trauma team). There is no rush to get him to the hospital, just "he got poisoned, deal with it".

That yanked me out of the game pretty hard. I can't stretch my disbelief that hard, sorry. For anyone playing beyond act 2, does it get better or do I have to put up with that sort of writing for the rest of the game?

You obviously haven't played too much further then that. No one believes it except the slubs.
