Has it been patches or just some random glitches, some things (uncommon grenades, the Lemon soft drink, Maxdoc 3) have previously been able to be disassembled, but now can't be?

Previously one of the different soft drinks (blue) stopped being able to be, but then later it started working again.

If it is a glitch, is there a way to undo it?

Yep, it's a known one. Apparently happens to all stackable items, as you're describing. What seems to help is consuming one of the items or an item from another stack (food/health) or dropping entire stack and picking up it again. Or a combination of those.
