Phantasy Star IV
Buck Rogers : Countdown to Doomsday
Shining in the Darkness
Desert Strike (so shoot me, it WAS a good game, back then :P)
Starflight
Wonderboy in MonsterWorld
Shadowrun. By far.

Runner up goes to Splatterhouse 3.

deshadow52: I would have to say M.U.S.H.A is the best.
One of the best shoot'em up I have ever played.

choconutjoe: Toejam and Earl!

That game is in a genre of it's own.
This!

And Golden Axe 2!
Phantasy Star II
NHL, Golden Axe & Shining Force ii
In no immediate preference: Gunstar Heroes, Alien Soldier, Sonic 3 and Knuckles, Ristar, Vectorman, Golden Axe, Phantasy Star IV, Toejam and Earl, and Wonder Boy In Monster World.

some others i consider good would Sonic 3 and Knuckles, Wonder Boy in Monster world, Last Battle/Hokuto no Ken and Robocop vs The Terminator
I only had the genesis for about a 18 months before I sold it for rent money. I missed the later years of it.

Revenge of Shinobi
Mickey Mouse and the Castle of Illusion
Ghouls 'N Ghosts.

Never cared for Golden Axe. I found it repetitve and dull. I know it's not supposed to be deep or anything, but even for an arcade beat 'em up I didn't like it that much

King's Bounty probably for me, just based on time spent. I played it so much that the rental shop eventually let me buy it for the same price they were renting it for. :D
Sonic 2
Mortal Kombat 1 + 2

And quite a few of the games listed from others.
