You should try Europa 1400: The Guild(it's called The Guild Gold here on gog).
There is also The Guild 2, but they said it isn't very good. You might try the demo first and see for yourself.
Both games are here on gog.

surgeon simulator. you play as a surgeon. in space. totally legit!

edit: the above was a joke. game dev tycoon, however, is a cool career simulator. check it out!
