I have a few on here but what are some of the hidden gems I don't know about?
(I have Fallout 1/2 and Brotherhood, MoO1/2.)

I consider Call to Power 2 an underrated gem. Also Nexus the Jupiter Incident is worth looking into and a lots of other games, depending on what you like.

You shouldn't forget the D&D games (BG, NWN, IWD, Torment etc), the Heroes of Might and Magic series, Wizardry 8.
Also if you like city builders take a look at Caesar III, Pharaoh and Zeus. Ah and I almost forgot, you "have" to play Deus Ex :D
Some games that never got the attention that they deserved, and which I really like:
Sacrifice
Outcast
System Shock 2 (not really hidden these days, everyone knows that it is great, but it did sell poorly when it was first released)
Etherlords (2 more than 1)
Gateways
Knock-Knock
Original War
Pathologic
Dark Reign
If you like the Fallout games then you simply must get Arcanum. It's made by the same group, and while it has many flaws, it is one of the most interesting RPG worlds and has fantastic role playing freedom.
List of my favorite GOG games. Keep it mind it's highly subjective ;)

From the current promo I can wholeheartedly recommend:
The Guild
The Book of Unwritten Tales
The Book of Unwritten Tales: Critter Chronicles
For community members' hidden gems also check out this thread:

The Obscure Games Page
http://www.gog.com/game/one_unit_whole_blood Blood! if you in anyway like FPS games or horror games you must play Blood. Blows Duke Nukem right out of the water.

I'd also recommend

http://www.gog.com/game/clive_barkers_undying Clive Barker's Undying (Be warned it's hard. But nearly as good as Half-life)

http://www.gog.com/game/shadow_warrior_complete Shadow Warrrior

http://www.gog.com/game/crusader_no_remorse Crusader: No Remorse (The FMV cut scenes are surprisingly watchable)

http://www.gog.com/game/unreal_gold Unreal

http://www.gog.com/game/shogo_mobile_armor_division Shogo: mobile Armour Division. (Be warned: hard as balls!)

http://www.gog.com/game/duke_nukem_12 Duke Nukem 1&2 (surprisingly good shoot and jump games)

http://www.gog.com/game/symphony Symphony (a great game if you like music and colours)

And while these aren't obsure I'd still recommend them if you havn't played them

http://www.gog.com/game/prince_of_persia_the_sands_of_time Prince of Persia Sands of Time (Still looks beautiful even today)

http://www.gog.com/game/giana_sisters_twisted_dreams Giana Sisters (my Favourite platfomer to date)

http://www.gog.com/game/machinarium_collectors_edition Machinarium

and
http://www.gog.com/game/dust_an_elysian_tail Dust: An Elysian Tail I haven't played this one but it has furries so it must be good ;)

Deadly Premonition.

Sacrifice is, by far, the best game you've never heard of . It's the action-RTS game, the game that invented a new genre and that is still the golden standard for it .

Other than that, King of Dragon Pass, Eador:Genesis, Outcast(I don't really like it, but it's a good game nonetheless), Planescape: Torment, Shogo:Mobile Armor Division(only buy this one if you like FPS games), Sanitarium... Can't think of any others right now :)
Master of Magic!

The Spiderweb games (Geneforge, Avernum and Avadon) are pretty awesome too.
Battle Realms

Conquest: Frontier Wars
