SWTOR for all categories since that's the only MMORPG I've played to date!
Ultima Online. It got the entire genre really going and, well, it is Ultima!
The best in every aspect was and still is Everquest.
Too bad it's slowly dying away. If pvp servers were still alive, i'd still play it.

wizall: Eve Online by far, for me. I'm not a power gamer and that game allows for casual play more than any other MMO I've played. Casual play that sees consistent gains. I've tried quite a few, some for as much as 6 months, but nothing hooked me like Eve.
I would also like to point out that EvE online should be considered for:

Worse Community (there are some good people in EvE but the majority are fucking pricks)
For me had to be the original everquest prior to planes of power expansion. The game for me was fantastic got to meet a lot of people from around the world, one of the biggest if biggest worlds ever even before the expansions. The amount of times you had to wait for the ships to come in to reach other islands only 2 miss one by a minute and have to wait 20 mins for the next one. The game 3d ok not great now but hell at the start was great creating a character from any race and seeing it and meeting other people in 3d.
My favorite MMORPG was Guild Wars, because

a) it was released right when Diablo 2 was getting on my nerves
b) does not have monthly suscription fees (ok it has been some time for me)
c) bug fix patches were added very fast when something went wrong
d) it has a (more or less) realistic graphic style
e) I liked the game mechanics (aka you had to decide what skills you took with you on adverturing)

I wonder if Guild Wars is a ghost town now that Guild Wars 2 is out. Anyone know? I own it and should probably play it while the servers are still up. I did try it out briefly and it seemed fun but then I wandered off back to other games and forgot about it.

DieRuhe: Well, I just downloaded Everquest on Steam, so I'll see how it goes.
Beware its interface is a mess of seperate windows so you have to orginize it yourself or install a UI mod for your taste.
I always liked the EQ UI because I could easily set it up just the way I wanted it. I found it very customizable although there was a little learning curve getting used to it and how to do so. Once I had it set up just so it really enhanced my enjoyment of the game.

Humans fight giant cats. Cats bring fire. Human get burned and run away to snowy mountains where *midgets* kill their big man. Humans mad and make everyone else mad at them and go to live with other humans because no one likes them. Other humans religious crazies who bring lizards to punish the good humans. They run away to find out why the bad lizards have come. They go to find a bigger lizard who tells them about a bad man who they thought was a good man. Humans go to fight this bad man in volcano. The bad man is killed and everyone eats cake.

*Not sure if Dwarf is proper nomenclature for a six-year old*

Condense all MMO main campaigns in this fashoin, the one that sounds the most outrageous gets the prize of best lore.

The Prophecies zones have been a ghost town since the last expansion came out. I haven't played it since GW2 was released but you never know, it could be getting a nostalgia boost.

It's just an example for people to post what they like about an MMORPG, not that Guild Wars has a good lore.
My personal favorite: NEStalgia.
It would have to be the only MMORPG I've played to date: Runescape

I really can't argue with that, but it's what's attractive about the game. Or, rather, it's a byproduct of having a truly open system.. Every other MMO feels like it's on rails compared to it. If Goons and other trolls enjoy it, too, it's cool with me. I met some great people in the game to counteract the assholes. We found an area pretty remote and were left pretty well alone.
