I can't open the chest where manuscript is. There are four signs and a 'hint', but I really do not understand what the hint meant.

Yea, I don't understand the hints in any of the rooms either. All I did was try all the combinations until I found the right one. Thankfully I hit the right combination after like 10-15 tries.

You have to activate the four runes in a specific order. So if each rune has a number do:
1234
1243
1324
1342
1423
1432
2134
2143...etc until you hit the right combination. I don't remember what the right order is, sorry. You can either wait till someone else puts the combo up, or do it my way. Good luck!

I notice i didn't have Nekkar Warrior Blood for the ingredients. Is there any nekkar in Chapter III? What about the sewer, i notice lots of locked door at this city.

Pretty sure there are no Nekkers, I needed a Harpy egg and even though harpies spawn in act 3 they dont seem to drop the eggs.

You can decipher the meaning of the hint if you buy the rune books from the wizard who gives you the gargoyle quest. This room is a part of it.

If you bought the game on GOG, there's a guidebook among the goodies, by the way.

The puzzle is apparently randomized. A couple of the vendors in act 3 sell a set of books that contain the meanings of all the runes, but the description of the runes, and how it translates to the clues in each of the rooms can sometimes be hazy. It seems like a translation issue to me.

So basically, it will just involve some trial and error, and referring to the books to tell which symbol is which.

I don't know how i can't have Nekkar warrior bloods in my inventory because that monster is very common since Chapter I. I only have one Harpy egg not sure how it can make into my possession, that mean it is very rare. Maybe you can find it only in Chapter II.

But i did find Rotfiend in the sewer. And i see there are two guards guarding one of the sewer gate, but the door is locked. Maybe i'm out of luck for this quest.

I'm trying to open the chest to get the manuscript. Would I refer to rune book 1?

No I think the symbol descriptions are spread among all 4 books.
