Sorry to piggyback off of your post,

but I need assistance as well. I'm at my wits end. I've tried changing everything to the lowest possible setting but cannot get the QTE meter to fill. At the risk of carpal tunnel, I need help!

No. It's usually a problem of people trying to run the game on insufficient hardware. With everything happening in this particular fight, computers even slightly below minimum specs—or those that are above min specs, but running on higher settings than their computer can handle—can't register all of the clicks, so this particular QTE sequence can't be finished. Witcher 2 is one of the most demanding games out there at the moment and a lot of people don't realize that before jumping in, so problems like this are kind of inevitable. Not a bug, though.
