hi,
i have actually Witcher 3 v.1.10 + 16 dlc v.1.10 + Hearts Of Stone v.1.10
Does i need to unistall 16 dlc v.1.10 and Hearts Of Stone v.1.10 before applying the v.1.21 patch and reinstalling after that the patch the 16 dlc's v.1.21 and Heart Of Stone v.1.21?
Or just let my original game + dlc and extension v.1.10 apply the patch v.1.21 and installing Blood and Wine after?

You should be fine by just installing the patch (1.21). After that install B&W

I just applied Patch 1.21 to the base game and at the end of the install, it said it wanted V1.21 of the DLC pack and Harts of Stone.

Lots of downloading to do!

I have a similar problem. I downloaded the patch, I installed the patch, but Blood & Wine still claims my game needs to be patched.

Any chance you were using the older version of Hearts of Stone? I installed thusly: patch 1.21, then Hearts of Stone 1.21, and finally Blood and Wine. No troubles, done that way.
