According to youtube guides it should be in Skelige. I just checked and smith don't have this diagram. The moved it after 1.05?

Diagrams are in the loot tables. You'll need to be lucky to find a specific one, as they're never in the same place (except the quest related Witcher gears)

Problem is these diagrams are sold from NPC. I can buy any other diagram that is in the game except blank one.

seems it must have been moved then as that is the Blacksmith a bought the diagram from.
Go talk to Hattori as he now sells new diagrams that where added to his shop in 1.05 if he doesn't have it try the Master Armorer at Crow's Perch.

I bought it at a blacksmith in Skellige, west of the first town marker you find there.

However, its totally not worth having. The blank stone requires powdered black pearl, which you can buy for 450+ crowns per bottle.

Blech.

keira metz (herbalist in velen, involved in a ciri quest) sells alot of recipes too, maybe this is among them.
