Vitek: I saw it happen few minutes ago.

I made post in Eador forum 3 hours ago and now I came back to check if there are some answers. I was happy to see there was post made just 1 minute ago and thought myself lucky, but when I opened the game forum there was no new post since mine.
Yeah someone had made a 'help' topic in a Freespace thread and I replied to them. Then, several hours later, I got a reply indicator but there was nothing. I hope their post wasn't eaten as bevinator is suggesting as it was just the start of the help topic.

I had something similar happening to me, I think it happens when somebody answer to one of your post by mistake, then edit its answer to answer to another post. (by changing the quote number)

Happened to me a few times as well. The forum is playing mind tricks with us...

Because not all the threads I've noticed this for had hidden OPs - however, a greater percentage of them did seem to be on controversial topics than the standard population.
The OP needs to be downvoted to make thread appear greyed out.

It can happen when someone posts new post but not by clicking "New Post" button but instead clicks "reply" on your post and then deletes your quote. Because the reply message ins't generated by including quote but by clicking reply button.

Wishbone: This is usually caused by the OP being downvoted. Then the thread will always be gray, regardless of whether there are unread replies or not. 'Twas always so.

If an initial post gets downvoted enough to be grayed out, then that entire thread will always be grayed out. One downvote certainly won't do it. This can lead to weird stuff like having new replies in a gray thread, but that's how it works. It actually often makes me MORE likely to check a thread often, simply because I can't be sure if there are any new replies yet.

On the original topic, I'm convinced that SkyGOGNet really likes Heroes of Might & Magic, because there are phantom posts there almost all the time. The consciousness is almost strong enough to break through into the physical world now!

This is true, but in that case one still gets the little red wedge next to someone's post ... but I've gotten the little reply indicator when there was nothing in the thread except my post and the OP's.

Interesting, I hadn't seen many until recently in the forums I frequent, so I guess the intelligence is spreading.

I guess it is possible for some of them, that the OP had been downvoted enough, but then someone upvoted them so they weren't hidden, but the thread remains greyed-out? Because I could've sworn not all threads which were greyed-out had hidden OPs.

I believe this can be caused by someone clicking "Reply" on one of your posts, and then deleting what you wrote. I think this can result in the thread being marked with the "new replies" icon, but the post not being so.

Yeah but there is still the telltale red marker next to the post. I've gotten the reply indicator in threads where there are only two posts: the OP's and my reply to the OP. I got the reply indicator for that thread, clicked on it ... and nothing was there. No new posts.

Okay, yeah that sounds like a ghost post.
Remember that oftentimes the most obvious solution is the right one and in this case it is ghosts. Forum ghosts are actually quite common on boards like these. One of the most common is teenagers committing suicide and later on coming back to haunt the boards they used to roam in real life. Another one I've heard about several times is the tragic lover that lost her beloved to computer games. Beyond life she can't find peace and instead haunt the machines that took her dear away.
Usually they are mostly harmless and only cause small disruptions due to lost posts, strange forum behavior or posts with subtle changes in wording.
