Postal 2. I've pissed on a dude, chopped of his head while he was puking (vomit still comes through his neck) and pissed some more to his neck.

I know, everybody did that.
I created an "elf trap" in Dwarf fortress. There was a rout that all trade caravans had to take, over a moat and some drawbridges. This was originally intended to be a defensive system, but when I found lava I decided to have some fun with it. I created a small-ish walled section, with two entrances/exits, both which relied on drawbridges. Then I created a pumping system that would allow me to pump lava into that section (though it sadly would not contain it). When the elves came, I closed the area by closing the drawbridges, and then I gave a dwarf the order to pump lava into it. Take that you no good hippies who think that you are too good for my wooden items and things with blood on!

Severaly Skyrim quests come to mind. Namira was a especially evil one....
Carmageddon having pedestrians jump off a cliff, also Mortal Kombat Fatalities
When I was playing Fallout 3, I killed The Overseer, not realizing that he was Amata's father, so when she came to cry about it, I kept shooting her with BB's. (they were the only thing I had left x_x) That was when I realized that she can't fucking die, or maybe it was just a few too many BB's. I don't see how she wouldn't die, because I've gotten a good bit in, and I still haven't seen her since I left the Vault.

There is an quest later when you go back in the vault - you will meet her again then,

For me - I just did the "House of Horrors" quest in Skyrim, where you in the end have to kill an unarmed priest locked in a cage....

There is quite a lot of quest in Skyrim where you have to be not so nice....
