Descent 1 on Insane difficulty. It's hard enough just to learn how to fly the darn ship, much less do it when the first robot in the game can blow your ship to smithereens in a matter of seconds.
I agree with others on Jagged Alliance, and I think Commandos 2+3 is also pretty intense. Funny enough, lately it feels to me like all games are tough, so maybe I am losing my "edge", haha; I've been playing Valdis Story on the Easy setting, and I still lose now and again *shrug*; the game is versatile and fun, definitely worth playing.

The hardest game on GOG, however, might be Blade of Darkness, which I haven't played, but people call the predecessor of Dark Souls, so that's something to consider.

Theoclymenus: Clive Barker's Undying because of the (in my opinion) ridiculously difficult combat, which spoils an otherwise atmospheric, creepy game with a good story. The first "boss" (Lizbeth) just would not die when I played the game a few weeks ago. I don't usually consult walkthroughs because I consider it a form of cheating but after trying to kill her for about ten hours I needed to know whether it was even POSSIBLE. Eventually I managed it but didn't really understand why she had succumbed this time, and I did not enjoy the experience one bit.

There are also these creatures called "howlers" which positively launch themselves at you and are virtually impossible to dodge when they do so. There are some sections where they come at you in wave after wave and all you can do is run away blindly until you reach safety, having totally lost your bearings in the process. Such a disappointment as I was looking forward to (and otherwise enjoying) this "haunted house" experience. In the end I quit the game because in parts it was too much like an assault course when I had been hoping for a game which would spook me a out in a slightly less frantic fashion. Perhaps I'll give it another try some time but next time I will choose the lowest difficulty setting.
You mean there's no auto-dodge or auto-play buttons? What kind of crap is this?!?

Hardest part was House of Gorrors. Savant ending was anti-climactic.

I've gotta admit, there have been times when I wished I could be in a room alone for five minutes with the person responsible for some particular game I'm cussing and throwing stuff over. Probably a good idea it never happened. :)

Wizardry 7 is a real pain in the ass.
Witcher 2
MAX(Mechanized Assault and Exploration)... never could get past the first campaign level :P resigned myself to only playing skirmishes.
Oh, and there are some stages from Commandos : Behind enemy lines, and Beyond the call of duty that will drive anyone up the wall. (assassinate the general in BEL and the friggen train station in BTCOD)

TheEddevilish: Although I don't own it on GOG, there came a point in Spacechem where I just couldn't progress any further. Not without hours upon hours of planning and experimenting at least, but I didn't have the time or the willpower to carry on. Still an excellent game though :D
I think something like 2% of the people who own Spacechem have beaten it. I got stuck on a level few missions away from the end where I couldn't even see how it was possible to solve it. I had a lot of fun getting there though.
Super Hexagon or Dustforce if you want to complete everything.

Divinity II: Director's Cut is pretty hard in general and as a bonus it has random spikes in difficulty. Oh and if you don't scour an entire area for every single quest possible you WILL go into the next area completely underpowered, in which case, well...better hope you've invested a few skill points in summons and gotten your creature.
